Medical records

How we collect your medical records

We strive to make your visit to City of Hope as stress-free as possible. Prior to your first visit to City of Hope, we will handle obtaining your medical records and other documents related to your previous cancer treatments.

All you have to do is let your health care provider(s) know that City of Hope has authorization to collect this documentation on your behalf. To do this, simply fill out the Medical History and Authorization to Release and Disclose Information forms, in which you will provide information about your previous hospitals/physicians and authorize the release of private health information.

All of these forms are included in the information packet you will receive in the mail. You can also download and print them by clicking on the document below. You will need to have Adobe Acrobat Reader installed on your computer to open and print these pages.

Medical History and Authorization

Once complete, you can scan and email, fax or mail the forms to us. Complete instructions are included in the above file.
